CLASS SCHEDULING FORM

*Cancell ations inside 24hrs before scheduled start time will not be eligible for refund. Additions to class roster must be done 24hrs prior to start of class.

PAYMENT OPTIONS :

Course invoice will be emailed to class organizer (1) week prior to class start. Payments are due prior to class date. Payment types accepted: Check payable to CPRWA, PAYPAL (credit cards are also accepted through the PAYPAL system) and VENMO.

CLASS COMPLETION REQUIREMENTS:

All participants must attend all parts of the course. Late entries/early exit (15min, Exam) will not be eligible for class completion credentials. All participants must pass both a written and practical exam. Upon successful completion, participants will receive AHA OR ASHI certification good for (2yrs).
